Facility Request Form

You are invited to request facility space for an event, meeting or gathering by providing the following information.  Due to the numerous activities hosted by St. Malachy Church and School, we request information twelve weeks prior to the event.  We shall accept requests within twelve weeks and approve accordingly as space allows.  Thank you!.

Is the event recurring?*

If this is a fundraiser, has it been approved by the Finance Council?

Must have filled out the online Fundraiser Application. If not approved yet, facilities will not be scheduled.

If this event is not a fundraiser, has it been approved by a Parish Staff Member?

Director of Faith Formation, Youth Ministry, Father Danda, etc.

Is the event primarily for adults or children?

Is online registration required?

If yes, you will be contacted about the necessary information.

Are you in need of a kitchen?

Do you have technology needs

Examples include a TV, projector, speakers, computer, large screen, microphone, DVD/Blu-Ray player (please be specific as to the format), specific software i.e. Word, Powerpoint, etc., internet access, You-tube access, other websites. Please arrange to meet or speak with our technology coordinator at least one week ahead of your event to insure the technology equipment will work properly to meet your needs.
